SNORING
Snoring is a respiratory sound that is generated in the upper airway during sleep occurring mainly when breathing in. It can also occur during breathing out. It occurs in 10-12% of children and 24-40% of adults. Snoring is usually disruptive to the bed partner but is strong indicator and cardinal sign of Obstructive sleep apnea which can effect various organs.
OBSTRUCTIVE SLEEP APNEA SYNDROME (OSA)
Obstructive Sleep Apnea (OSA) is characterized by repeated episodes of either complete or partial airway narrowing that occurs in sleep that are terminated by awakenings. This results in poor quality of sleep, and patients wake up tired and unrefreshed regardless of the duration of their time in bed. Untreated OSA is an independent risk factor for the development of high blood pressure (hypertension). OSA is also frequently seen in patients with coronary heart disease, atrial fibrillation, stroke and Type 2 diabetes mellitus. Patients with severe OSA are at risk for developing pulmonary hypertension. When OSA coexists with other conditions like COPD and ischemic heat disease they can worsen these conditions. Various other conditions like mood disorders, cognitive impairment, acid reflux and erectile dysfuntion are often reported in patients with OSA.
INSOMNIA
Insomnia is a disorder characterized either by difficulty in initiating and/or maintaining sleep. The incidence is between 15-20%. It can either be short term, a likely result of stressors, or it could be long term. Detailed history and examination is necessary to ascertain the predisposing and precipitating factors of insomnia, and correcting these, with the help of cognitive behavior therapy and short term treatment with medications as needed, can result in restorative sleep.
RESTLESS LEG SYNDROME (RLS)
RLS is an urge to move the legs that worsens with periods of rest and is often partially relived with movement. It occurs predominantly in the evenings prior to going to sleep. Disrupted sleep is a common presentation. It can be familial, and often times there are multiple correctable causes that contribute to RLS.
NARCOLEPSY
Narcolepsy is a disabling condition associated with severe excessive daytime somnolence, hallucinations, sleep paralysis and cataplexy. Patients have low to undetectable amount of hypocretin (orexin). An overnight sleep study with a day time nap study (MSLT) and sometimes lab testing are needed to confirm the diagnosis.
EXCESSIVE DAYTIME SLEEPINESS (HYPERSOMNIA)
With Hypersomnia, patients suffer from excessive nocturnal sleep, daytime sleepiness or napping. This can be due to underlying medical or neurological conditions. Detailed history, lab work and further testing are required to evaluate this condition.
DELAYED SLEEP PHASE SYNDROME (DSWPD)
This is a condition where there is a significant delay in the major sleep period in relation to the desired or required sleep time or wake up time. Individuals with DSWPD demonstrate extreme tiredness in the mornings. This condition is more prevalent among adolescents and young adults.
